I understand I must complete the intake form at least 24 hours before my appointment or my appointment will be cancelled without refund.

The provider decides whether to issue a letter. Clinical denials are non-refundable (the fee covers their evaluation time).

I have previously booked an evaluation with WorkWell Evals.
